If you had a medical emergency, would you be prepared? Would the person you trust have the authority to make decisions for you? Join Stoughton Hospital social workers to learn about health care power of attorney. Immediately after the presentation, we will offer free confidential assistance in completing an advance directive. Please remember to bring addresses and phone numbers of your potential decision makers.
